Pantoprazole
It is a proton pump inhibitor and containing antacid activity. Pantoprazole is white to off white solid crystalline powder, which is freely soluble in water, very slightly soluble in phosphate buffer at pH 7.4 and insoluble in hexane and sodium. The moleculer weight of pantoprazole is 383.4g/mol. Melting point =199-150⁰c and Boiling point =586.9⁰c at 760mmHg. It is not recommended during pregnancy. It is also known as Pantoloc ,Pantozol, Protonix, Pantopraz olum, Pantoprazol, Panyocid etc.
■ Structure:-
The moleculer formula of pantoprazole is,
[C16H15F2N3O4S]
6-(difluromethoxy)-2-[(3,4-dimethoxypyridin-2-yl)methyl sulfinyl]-1H-benzimidazole
■ Mechanism of action:-
Pantoprazole is binds with H+/K+/ATPase enzyme at the surface of the gastric parietal cell, because it is a proton pump inhibitor. Due to this both basal and stimulated gastric acid secretion, irrespective of the stimulus are inhibited. The binding of H+/K+/ATPase effectively inhibit the acid secretion.
■ Uses:-
It is used in the treatment of,
- Duodenal ulcer.
- Erosive esophagitis.
- Gastroesophageal reflux disease.
- Zollinger-ellison syndromes.
- Helicobactor pylori infections.
■ Dosage:-
The usual adult dose for duodenal ulcer is, 40mg orally once a day. The dose is increased every 12 weeks by 40mg to a maximum 120mg per day for 28 weeks.
The usual adult dose for erosive esophagitis is 40mg orally once a day for 8 weeks. The maintainance dose is 40mg orally once a day for 12 months.
The usual adult dose for gastroesophageal reflux disease is 40mg orally once a day for 7-10 days.
The usual adult dose for Zollinger-ellison syndromes is 80mg orally every 12 hour or twice a day after meal.
■ Side-effects:-
The major Side-effects of pantoprazole is,
- Headache.
- Diarrhea.
- Constipation.
- Stomach pain.
- Heartburn.
- Fast/slow/irregular heart beat.
- Joint pain.
